中文摘要: 本研究突破过去多模态影像学简单结合使用的局限,解决传统影像学指标无法准确、多维度反映肝硬化结节微血管循环改变以及难以有效结合多指标显著提高结节癌变预测效能的关键问题。该项目拟:(1)对肝硬化结节可疑早期癌变患者进行超声造影、磁共振普美显动态对比增强及体素内不相干运动成像检查,获得多种影像定量参数;(2)对多模态定量参数进行直方图分析,获取多个维度的大量影像学特征指标,并筛选出最能反映肝硬化结节早期癌变病理学变化的特征指标;(3)基于多特征指标建立多元logistics回归模型,预测临床肝硬化结节病例是否癌变,并通过病理对照验证其临床有效性。本项目的研究结果将:(1)为肝硬化结节早期癌变的诊断提供更敏感的影像学特征指标;(2)对多模态影像的结合应用方法进行尝试以提高诊断早期肝癌的精准性,并为未来多模态联合研究提供经验参考。

英文摘要: The study breaks the limitation of simple combination of multi-modality imaging in the past, and solve the key problems that conventional imaging indices cannot accurately and multi-dimensionally reflects microcirculation changes of the cirrhotic liver nodule, and it is hard to efficiently combined multiple indices to significantly improve performance in predicting nodule carcinogenesis.This study plans to :(1) apply ultrasonic contrast enhanced imaging, MR contrast enhanced imaging with Gd-EOB-DTPA and intravoxel incoherent motion imaging.to patient with early carcinogenesis of nodules in the cirrhotic liver , and acquire multiple quantitative imaging parameters;(2)acquire large amount of multi-dimension imaging features from histogram analysis of the quantitative parameters, and select imaging features with the highest sensitivity in reflecting pathological changes of nodules carcinogenesis in the cirrhotic liver;(3)establish a multi-variable logistics regression model based on multiple imaging features to predict the existence of carcinogenesis of nodules in the cirrhotic liver, and validate its clinical effectiveness with reference to pathology results.This project intends to: (1)provide the most sensitive imaging features in diagnosis of the early carcinogenesis of nodules in the cirrhotic liver;(2)attempt to jointly utilize multi-modality imaging methods to improve the accuracy in early diagnosis of hepatocellular carcinoma, and will provide experience for future jointly study of multi-modality imaging.
