中文摘要: 中医脉诊历史悠久,但“脉理精微,其体难辨,在心易了,指下难明”的特点影响着它的传授与发展。充分揭示脉象的生物学基础,明确不同脉象的生物学指标,进行客观化描述,才能实现脉诊更好地传承与发展。故本课题在前期研究基础上,将超声瞬时波强(WI)技术引入脉象客观化研究,利用WI技术在可视界面下自动跟踪血管壁,实时检测动脉任意点的瞬时管径变化、血流流速变化,计算瞬时加速度波强、减速度波强、负向波面积等指标,以全面评估不同脉象形成时心脏及血管功能的特点。通过检测桡动脉WI各参数,定量分析心动周期不同阶段,不同脉种桡动脉的位置、管径、血流流速、流量、动能、势能,力学等的变化,分析脉象产生的机制;并将WI技术与脉象仪技术联合,同步观测不同检测点(寸口、人迎、趺阳)动脉的两种技术各检测指标,统计分析之间的区别及联系。期望能为脉象形成机理的阐明提供更多信息,为脉象客观化研究提供新思路和新途径。

英文摘要: Pulse-diagnosis in Traditional Chinese Medicine (TCM) has a very long history but it can’t be developed and inherited easily. The character of pulse-diagnosis in TCM is the major obstacle for inheriting, developing and promoting of the sphygmology of TCM. The character is that the pulse is very subtle and can’t easily be distinguished and described. Pulse-diagnosis method in TCM can be inherited and developed very well only by fully revealing the biological basis of sphygmology, clarifying the biological indicators of different kinds of pulse manifestations and objectively describing them. This study, basing on preliminary studies, takes advantage of ultrasonic wave intensity (WI) technology to study the sphygmology of TCM objectively. WI technology can automatically trace the vessel wall under the visual interface and detect the instantaneous changes of diameter, flow velocity of the artery at any site of circulation. Then the W1, W2, and NA will be calculated and analyzed to show the information about the dynamic behavior of the heart and vascular system and their interaction with different kinds of pulse manifestations. Through detecting radial artery of WI technology parameters, the position, diameter, flow velocity, kinetic energy, potential energy and mechanical characters of radial artery, during different stages of the cardiac cycle, will be quantitatively analyzed and the mechanism of pulse manifestations will be analyzed. Besides, WI technology and pulse detecting instruments will be jointly used to simultaneously observe the difference between these two kinds of technology in different detecting points (cunkou, renying, fuyang). Then the indicators will be statistically analyzed to find out the differences and relations. By those ways, we hope to provide more information about the developing mechanism of pulse manifestation and provide new idea and new way for objectively study of pulse manifestation.
