中文摘要: 原发性肌张力障碍病因不明,治疗效果欠佳,临床表现多样,以颅颈部肌张力障碍最为常见。在肌张力障碍造成的严重危害中,非运动症状如抑郁占据十分重要的地位,我们前期研究显示高达49%的原发性肌张力障碍患者伴抑郁。各种证据表明抑郁是原发性肌张力障碍的内源性表现,且会加重运动功能障碍,显著降低患者的生活质量,研究原发性肌张力障碍伴发抑郁的机制具有重要意义。近年来的研究成果表明抑郁的发生是由于情感网络异常所致;而既往研究和我们的前期研究发现原发性肌张力障碍存在累及皮层和皮层下、运动和非运动多处脑区的结构和功能异常,可能在不同程度上影响了情感网络的节点而引发抑郁,但国内外在此方面的研究十分匮乏。因此,本课题在前期工作的基础上,拟联合结构和功能磁共振技术首次研究原发性肌张力障碍抑郁发生的情感网络机制,及其与运动网络的关系,将为深入研究原发性肌张力障碍病理机制提供新线索,为其治疗提供新思路和新靶点。

英文摘要: Primary dystonia has variable clinical manifestations with craniocervical dystonia most common, its etiology is unclear and treatment effect is unsatisfactory. Non-motor symptoms such as depression plays an important role in the serious harm which dystonia brings to patients, our preliminary work shows the prevalence of depression in primary dystonia up to 49%. Cumulative evidence has proven that depression is an intrinsic feature of primary dystonia, and it would impair patients’ motor ability and decrease the quality of life. It is remarkable meaningful to study the mechanism of depression in primary dystonia. Recent researches have shown that depression results from dysfunction of emotional network. Functional and structural abnormalities of multiple brain regions covering cortical-subcortical and both motor and non-motor regions, revealed by previous studies and our preliminary studies, may affect nodes of emotional network and lead to depression. However, research about this aspect is scarce at home and abroad. Therefore, based on the preliminary work, we will first combine the structural and functional MRI to investigate the emotional mechanism of depression and its relationship with motor network in primary dystonia. This project will provide new evidence for the pathological mechanism of primary dystonia and provide new ideas and novel targets for the treatment.
