中文摘要: 早期明确病原体是临床有效控制菌血症的关键,现有细菌鉴定方法耗时过长,亟待研究快捷准确的新型检测方法。细菌挥发性代谢产物(mVOCs)是细菌在生命活动过程中产生的一大类小分子挥发性产物,我们前期应用自主研发的电喷雾萃取电离质谱(EESI-MS)技术检测mVOCs,并通过mVOC指纹图对三株相似度很高的球菌进行了有效区分。据此我们提出:不同病原菌存在独特的mVOCs指纹图,可通过EESI-MS检测予以快速鉴定。本研究拟采集血培养阳性报警瓶上层气体或菌落,应用EESI-MS等技术检测标准菌株及临床分离株mVOCs,通过主成分分析等获得不同病原菌特征性质谱标志物或mVOCs指纹图,利用多级串联质谱技术确定标志物的分子结构式,建立不同病原菌的mVOCs指纹图库,并分析该方法在周转时间上对于传统细菌鉴定方法的优势。旨在研发一种基于mVOCs指纹图的菌血症病原体快速鉴定新方法,为早期控制菌血症提供依据。

英文摘要: Early identification of the pathogenic bacteria is critical for successful control of bacteremia, existing bacterial identification methods takes too much times, rapid bacteremia detection method urgently need to be updated. Microbial volatile organic compounds (mVOCs) are a large class of small molecule volatile metabolites produced by bacteria in its biological processes. Previously, we used a new mass-spectrometric technology, which were developed by our team, to detect mVOCs, and successfully identified three strains of cocci based on mVOC fingerprints. In the study, we proposed that the existence of different pathogens characteristic mVOCs fingerprints.We will collect the headspace gas in positive blood culture bottles (take bacterial colony as samples if necessary), extractive electrospray ionization mass spectrometry (EESI-MS) will be applied to detect mVOCs from standard strains and clinical isolates; principal component analysis(PCA) will be performed to obtain molecular markers or mVOC fingerprints of different pathogens; multi-stage tandem mass spectrometry will be used to determine the molecular structure; create mVOC fingerprints gallery of different pathogens; and finally, analysis of the advantage of this method over traditional identification methods in terms of turnaround time. Aims to develop a rapid pathogen identification of bacteremia based on mVOC fingerprints, provide experimental evidence for early symptomatic control bacteremia.
