中文摘要: 肺炎支原体是小儿社区获得性肺炎的主要病原之一,其能量代谢和免疫功能的紊乱状态,导致了疾病容易迁延不愈或反复发作,并容易演变为哮喘及慢性咳嗽。清肺通络方是上海市龙华医院儿科治疗支原体肺炎(MPP)的自制制剂,功效清宣肺卫,通络活营,前期已证实有较好的临床疗效,可降低MPP患儿升高的血清IL-4、IL-17水平,改善MPP小鼠的血瘀状态。 本课题以中医学的"营卫学说"为指导,提出MPP发病"卫闭营瘀"的病机,以能量代谢与免疫炎症之间的相互作用为机体稳态调节的核心机制,利用1H NMR技术对MPP小鼠的血清代谢组学进行研究,筛选MPP的代谢标志物,并与TH1、TH2、TH17细胞及炎症因子IL-4、IL-6、IL-17等进行相关研究,探索能量代谢与炎症因子的相互作用,进一步认识MPP的微观物质基础,将"代谢-免疫"交叉点作为中药清肺通络方干预治疗MPP的靶点,评价其疗效机理。

英文摘要: Mycoplasma pneumonia is one of the main pathogeny of pediatric community-acquired pneumonia, the state of energy metabolism and immune function disorders leading to the disease easy to delayed healing or recurrent episodes, and easily evolve into asthma and chronic cough. Qingfei Tongluo formular is preparation of pediatric department of shanghai Longhua Hospital treating MPP, clearing the lung guard and dredging collaterals, which has better clinical efficacy. We had confirmed that it can reduce the elevated serum IL-4, IL-17 levels of MPP children and improve the blood stasis of MPP mice. The topic take the TCM "Ying-Wei" doctrine as guide, raise the pathogenesis of MPP as "the guard closed and the Ying stasis". Take interaction of metabolism and inflammation as core mechanism of the body homeostasis, use of 1H NMR techniques to study serum metabolomics of MPP, screening its metabolism markers, and TH1, TH2, TH17cells, the cytokines IL-4, IL-6, IL-17, IL-1β, IL-18, IL-13 in association studies to explore the interaction of cytokines and energy metabolism, a better understanding of microscopic material basis of MPP. Take intersection of metabolism-immune as targets treating MPP, evaluate the efficacy mechanism of Qingfei Tongluo formular.
