中文摘要: 糖尿病所引起的中枢神经系统损害已得到证实,既往临床数据和神经影像研究均已证实2型糖尿病(T2DM)患者脑部的结构和功能都存在异常,并发现神经影像的相关参数可成为T2DM认知障碍的潜在标记物。但目前相关研究都集中于横断面,且模态和算法单一,难以观测T2DM患者脑部及相关认知功能障碍的动态变化规律。因此,本研究拟收集正常人(n=100)和T2DM患者(n=100),每12个月定期随访,进行神经量表、血生化检查和多模MR扫描。首先通过内在连接网络(ICN)的时间模式和空间模式,评估组间差异;再通过基于贝叶斯网络的多变量分析方法,从海量影像数据中找出T2DM认知障碍潜在神经影像学标记;最后通过多模MR的随访研究,从脑结构、功能和网络等方面来研究T2DM患者脑部异常改变的动态演变规律,综合影像学数据、神经生理数据、临床行为学数据,寻找影响T2DM认知障碍早期诊断的高特异性和高敏感性生物学标记。

英文摘要: This proposal focuses on investigating possible mechanism underlying type 2 diabetes (T2DM)-related cognitive impairment using functional MRI technique. Related clinical data and our previous studies have shown that MRI based neuroimaging can be potential biomarker for diabetes-related cognitive impairment. We aim to explore the imaging biomarkers for the early diagnosis and treatment for diabetes-related cognitive impairment. Our goal is to find specific brain imaging biomarker for T2DM. In this study, both, cross-sectional and follow-up imaging data of T2DM patients will be collected. Various imaging parameters will be compared between T2DM patients (n=100) and healthy controls (n=100) to explore the brain alterations in diabetic population. Intrinsic connectivity networks (temporal and spatial patterns) and Graphical Model Based Multivariate Analysis (GAMMA) will be applied in the data analysis to identify the sensitive and specific imaging marker for T2DM patients. Finally, these imaging parameters will be regressed against different T2DM-relatedclinical variables and cognitive performance to explore the possible mechanism underlying the cognitive decline related to T2DM.
