中文摘要: 本研究分为3个部分,第一部分是以安慰剂为对照,采用多中心、随机、双盲方法,运用GC-TOFMS技术,研究芪明颗粒对非增殖期DR气阴两虚虚代谢组学的影响(芪明颗粒60例,安慰剂60例),结果显示芪明颗粒能改善受试者气阴两虚证候,减轻受试者全身症状,提高受试者视力。其作用机制为改善受试者能量代谢状态,使细胞处于较好的营养、代谢状态,潜在生物标记物可能为氨基酸和有机酸。第二部分是以健康人30例为对照,纳入有阳虚DR患者30例,无阳虚DR患者 30例,采用GC-TOFMS技术获得DR病证相关血浆代谢指纹图谱,分析DR分期、分证的差异性分析,寻找与DR疾病进展相关潜在生物标记物,结果显示DR兼阳虚证与非阳虚证之间,增殖期与非增殖期之间在血浆代谢组学方面存在差异;相应载荷图显示DR兼阳虚证和增殖期的可能潜在生物标记物均为氨基酸、有机酸,阳虚与能量代谢物质密切相关。第三部分采用有阳虚和无阳虚DR患者各40例,正常人21例,采集证候信息并用PCR的方法研究其的VEGF基因特点,比较发现糖尿病视网膜病变VEGF基因rs1413711位点的多态性与阳虚病机间具有密切相关性。DR证候分布特征与VEGF多态性

英文摘要: This study divides into three parts.The first part is based on placebo-controlled, with multi-center, randomized, double-blind method and GC-TOFMS technology to research the effect of metabonomics in Qiming Granule treating non-proliferative diabetic retinopathy (NPDR)and both qi and yin deficiency. It includes Qiming Granule group (n=60)and control group(n=60). The result showed that Qiming Granule can improve the syndrome of patients with both qi and yin deficiency, can reduce systemic symptoms and improve their vision. The mechanism of action is to improve the energy metabolism of patients, which makes the cells are in good nutrition and metabolic status. And the potential biological markers may be amino acids and organic acids. The second part is based on healthy people-controlled(n=30), yes/no yang deficiency patients(n=30). Metebonomics fingerprints of the associated plasma in DR were gained by using the GC-TOF MS technology ,it analyzes distinction between DR phase and Zhengs ,and find potential biological markers of each DR stages.The results showed that there are differences in plasma metabolomics between yang deficiency and non yang deficiency in DR, also between proliferation DR and non-proliferation phase. The corresponding diagram shows that the potential biological markers of DR with yang deficiency syndrome and the proliferation DR are amino acids and organic acids. Yang deficiency is closely related with metabolism of energy and material. The third part has yes/no yang deficiency patients with DR each 40 cases, another 21 cases are normal. In order to study characteristics of the VEGF gene by collecting syndromes and PCR, The study finds that there are some closely relationships between the rs1413711 situs's polymorphism of Gene VEGF and the pattern of diabetic retinopathy.The distributing tendency of the pattern character is similar to the rs1413711 situs 's polymorphism of Gene VEGF.It hints that the 'Yang deficiency'syndrome is the important pathogenesis of DR has the molecular organism accordance.
