中文摘要: 为辅助临床实施肿瘤精确放疗并保护危及器官,本申请拟通过研究包含呼吸运动信息的四维序列图像特征来定量分析胸腹部肿瘤和危及器官随呼吸运动变化规律。研究内容包含如下四方面:(1)研究联合空域和时域信息的形变配准算法实现4D-CT中不同时相的呼吸相关序列图像配准,从而获取肿瘤靶区的运动规律并辅助定位;(2)通过构建边缘保护多尺度空间,并结合基于B样条的自由形变网格模型设计配准算法,用于实现计划日4D-CT和治疗日3D-CT图像之间的配准,重建放疗当日4D-CT图像;(3)研究临床可行的4D-MRI技术,通过基于原始图像片层内部的身体区域像素变化提取呼吸信号并计算各个呼吸时相,使用多项式拟合和主成分分析法对形变域优化,实现4D-MRI中伪影去除,并通过形变配准算法实现增强;(4)将4D-CT和4D-MRI技术实施临床验证和应用,以放射性肝脏损伤为例,研究基于形变配准算法的点对点剂量叠加方案。

英文摘要: Aiming for assisting precise radiotherapy and protecting organs at risk, respiratory motion with abdomen-thorax is analyzed quantitatively by studying the characteristics of four dimensional sequence images. Our research mainly focuses on four parts. Firstly, deformable image registration algorithm using spatial and temporal information jointly is proposed for 4D-CT with different respiration related phases in order to obtain the tumor motion. Secondly, edge-preserving scale space is constructed for multi-scale decomposition of free breathing 3D-CT images in treatment day, and then aiming for reconstructing the 4D-CT in treatment day, the deformable registration algorithm is implemented on 4D-CT in planning day and 3D-CT in treatment day by using B-spline based free form deformation model. Thirdly, based on the internal pixels changing in body area with the original images, the respiratory signal is derived from clinic practical 4D-MRI, and then the respiratory phase is calculated based on the respiratory signal. Furthermore, for removing the artifacts in 4D-MRI, the deformation fields are optimized by using polynomial fitting and principal component analysis, and deformable registration is followed for enhancing the tumor and soft tissues. Finally, clinical application for liver cancer patients is studied based on 4D-CT and 4D-MRI technique, point to point dose accumulation is implemented based on the deformable registration for radiation injury prediction with hepatic organ. Both of phantom and clinical patients are studied for verification and evaluation.
