中文摘要: 听觉加工异常可能是导致精神分裂症幻听症状的重要原因。以往研究多关注患者在听觉皮层内的抑制障碍,或者与前额叶的协同效应异常。本研究组前期研究则发现,楔前叶与听觉皮层的抑制连接缺陷可能是导致异常听觉体验的重要因素。该项目采用脑电Gamma共振指标观察精神分裂症在听觉皮层和楔前叶的局部抑制功能,同时应用功能磁共振技术探索两个脑区在近听觉阈任务下的功能连接特征,并整合局部脑区的脑电生理特征与脑区之间的功能连接信息,多维度探讨楔前叶与听觉皮层连接缺陷在精神分裂症发病机制中的作用。本研究还引入重复经颅磁刺激(rTMS)治疗患者左侧颞上回,通过磁共振内即刻干预和8周长时程干预两种方式,观察患者局部脑区Gamma共振与功能连接的纵向变化,揭示楔前叶与听觉皮层之间抑制调节的因果关系。如能够获得预期研究结果,将为阐明精神分裂症听觉加工异常的病理机制提供重要线索,并为rTMS治疗精神分裂症寻找可靠的生物学靶点。

英文摘要: Abnormal auditory processing in schizophrenia may be an important cause leading to the auditory hallucination. Previous studies pay more attention to impaired internal inhibition in auditory cortex, or abnormalities in synergistic effect between prefrontal lobe and auditory cortex. Evidence from our group suggested the weakened inhibited relationship between auditory cortical and the precuneus be the important factor leading to the abnormal auditory experience in schizophrenia. This project will use the Gamma oscillation index in EEG examination to observe the local inhibition in auditory cortical and the precuneus respectively, and explore the functional connectivity between auditory cortical and the precuneus in the application of magnetic resonance technology with the near threshold auditory task in schizophrenia patients. This study will integrate local brain electrical characteristics and spatial function connection information, to explore the precuneus and auditory cortex connection defects in multiple domains of schizophrenia’s pathogenesis. We further utilize repetitive transcranial magnetic stimulation (rTMS) intervention on left superior temporal gyrus, with immediate intervention as well as 8-week protocols respectively, to detect the longitudinal changes of Gamma in local brain regions and that of brain functional network as well as the causal relationship between auditory cortical and the precuneus. If we the anticipated results were gained, we will not only obtain important clues to explain pathological mechanism in auditory processing abnormality in schizophrenia, but also provide a reliable biomarker for rTMS treatment in schizophrenia.
