中文摘要: 社区糖尿病健康管理是控制糖尿病日趋严峻的流行趋势的有效手段。我国社区糖尿病健康管理尚处于起步和初级阶段,管理机制不够完善,在效果评价上缺乏定量评价模型。本研究运用系统动力学的观点和建模方法构建社区糖尿病健康管理机制的系统动力学逻辑模型和量化模型。分析糖尿病在健康状态、糖尿病前期、临床期、并发症期以及死亡的疾病演化和转归过程中,社区糖尿病健康管理的作用环节、作用方式与作用效果之间的逻辑关系。确定社区糖尿病健康管理机制与疾病过程、环境因素、卫生资源配置、健康管理措施、医疗保障、医疗负担等相关因素之间的定量关系。并通过健康管理政策模拟与干预实验,评价健康管理机制,促进其优化,为有效的社区糖尿病健康管理提供政策建议与决策支持。本项目首次将系统动力学建模方法应用于中国人群糖尿病健康管理问题的研究,该方法学体系还可运用到其他慢性病健康管理机制研究与评价,具有一定的创新性和推广价值。

英文摘要: Community diabetes mellitus management is an effective means to control growing prevalence of diabetes. Community diabetes mellitus management in China is still in its infancy. The mechanism still has many imperfections and is lacking in quantitative evaluation models. In this study we will use system dynamics modeling methods to establish logical models and quantitative models of community diabetes mellitus management mechanism. We will define the logic loops of community diabetes mellitus management action segment, action modes and action effects during the progress in diabetes from health to prediabetes, diabetes, complications and death. We will also make clear the quantitative relationships among disease progress, social surroundings, health resource allocation, health management methods, medical security and medical burden. By conducting experiments of diabetes mellitus management strategies simulation and intervention, the management mechanism will be evaluated and optimized, and policy proposal will be proposed to achieve efficient community diabetes mellitus management. System dynamics modeling methods are first used in community diabetes mellitus management in China in this study, and can be used generally in other chronic disease management. The study has some certain innovation and promotional value.
