中文摘要: 胰岛素抵抗是糖尿病心肌病(DCM)发生发展的关键环节,其调控机制尚未完全阐明。PDCD4是近年来新发现的炎症凋亡基因,我们前期研究发现其表达缺失能够明显上调胰岛素敏感性从而显著改善胰岛素抵抗,但是在DCM中PDCD4与胰岛素抵抗的作用及其作用机制,目前还不清楚。为了探讨PDCD4在DCM胰岛素抵抗发生发展过程中的重要作用,本研究拟以高脂饮食联合小剂量链脲佐菌素诱导建立与人类2型糖尿病临床特征相似的动物模型,综合利用siRNA以及分子生物学、病理学、小动物超声等技术探讨PDCD4表达沉默和过表达对胰岛素敏感性的影响,观察PDCD4表达干预后对DCM心脏结构和功能的变化;然后体外分离培养原代心肌成纤维细胞和心肌细胞进一步在细胞生物学、组织学和整体功能水平深入阐明PDCD4对DCM胰岛素抵抗的作用机制,这些结果将为进一步明确DCM的治疗靶点,改善DCM患者的预后提供新的治疗依据和研究思路。

英文摘要: Insulin resistance is a key factor on the development of diabetic cardiomyopathy (DCM) and the mechanism has not yet been fully elucidated. PDCD4 has been recognized as a novel inflammation and apoptosis gene in recent years, and lack of expression could significantly improve insulin sensitivity and increase energy metabolism in our preliminary study. In order to explore the relation between PDCD4 and insulin sensitivity in DCM, we would establish type 2 diabetes animal models with a high-fat diet in combination with small dose streptozotocin. The effect of PDCD4 on insulin resistance would be investigated using gene silencing and gene overexpression by siRNA, molecular biology, pathology, animal ultrasonic technology. Then primary cardiac fibroblasts and myocardial cells are further cultured by cell biology, histology for clarifying molecular mechanism. These results would suggest therapeutic targets on DCM and provide new treatment method to improve prognosis of DCM.
