中文摘要: 移植肾急性排斥是影响移植肾长期预后的重要原因,但临床上缺乏有效的无创性动态监测与诊断手段。颗粒酶B、补体C4d分别作为体内细胞免疫与体液免疫反应激活的标志和产物,对移植肾急性排斥具有重要的诊断意义。本研究将利用抗大鼠颗粒酶B单克隆抗体与抗大鼠C4d单克隆抗体分别制备以颗粒酶B或C4d为靶向的磁共振对比剂,对急性排斥、急性肾小管坏死及对照组肾移植模型大鼠进行移植肾磁共振成像,观察并比较上述对比剂对3组移植肾的磁共振增强效果,评价该靶向磁共振技术对移植肾急性排斥的诊断敏感性与特异性,以及对大鼠体内细胞免疫、体液免疫激活程度的反映效果,验证其作为无创、有效的移植肾排斥诊断方法及体内免疫活化状态的检测手段的可行性。

英文摘要: Acute rejection is the important factor of long-term prognosis of renal allograft. However there is no effective, non-invasive method of dynamic monitoring and diagnosis. Granzyme B and complement C4d are the products of cell-mediated immunity and humoral immune response activation and have important diagnostic significance. In this study, we will use the anti granzyme B monoclonal antibody and anti C4d monoclonal antibody to prepare granzyme B or C4d targeted MRI contrast agent, and perform MR imaging in rat models of acute rejection, acute renal necrosis and normal control with these agents. The intensity of MR signals will be compared between groups to evaluate the sensitivity and specificity of the granzyme B or C4d targeted MR imaging in diagnosis of acute rejection or acute renal allograft necrosis. The reflection of cell-mediated or humoral immune response in vivo by this targeted MR imaging will also be tested.
