中文摘要: 背景:飞秒激光角膜透镜移植是一种在角膜基质层间植入预设屈光度的组织透镜来矫正屈光不正的潜在途径,其屈光效应预测性有待提高,生物力学变化规律有待揭示。研究目的:通过动物实验建立同种异体飞秒激光角膜透镜移植后角膜屈光度与透镜参数之间的量-效模型;揭示角膜生物力学变化规律,评估生物力学变化对屈光效应的影响;通过检测透镜移植后角膜创伤修复反应,探讨同种异体飞秒激光角膜透镜移植的愈合特点,并为屈光效应和生物力学效应的变化提供相关因素分析。研究方法:对兔眼进行透镜移植,通过角膜地形图测量角膜屈光参数;Corvis ST、ORA测量角膜生物力学效应;活体角膜共聚焦显微镜观察角膜组织结构改变;TUNEL法检测角膜细胞凋亡,免疫组化技术检测Ki-67及CD11b的表达。预期研究结果:初步建立屈光度量-效模型并揭示生物力学变化规律。研究意义:为同种异体飞秒激光角膜透镜移植在临床中手术参数设计提供理论依据。

英文摘要: Background: Femtosecond laser assisted corneal lenticule transplantation is an innovative surgical modality to correct ametropia. In the procedure, an allograft of corneal lenticule with designed refractive power is implanted into the recipient corneal stromal bed. Studies need to be carried out to evaluate the refractive predictability and the resultant corneal biomechanics related to the surgery. Purpose: To establish a dose-effect model between postoperative corneal refractive outcomes and lenticule parameters; to determine postoperative corneal biomechanics properties with their potential impact on refractive outcomes; to evaluate postoperative corneal wound healing processes and its correlation with refractive outcomes and corneal biomechanics. Methods: Allogeneic lenticules are implanted in rabbit corneas. Corneal refractive parameters are measured using corneal topography before and after this procedure. Corneal biomechanical parameters are tested using Corvis ST and ORA. Confocal microscopy is performed to observe corneal morphological changes. TUNEL assay is carried out to test corneal cell apoptotic status and the expression of Ki-67 and CD11b is tested by immunohistochemical techniques. Expected Results: to create a dose-effect model between refractive outcomes and lenticule parameters; to elucidate the properties of postoperative biomechanical changes. Research Significance: The outcomes of this project will provide theoretical basis for surgical designing of allogeneic femtosecond laser assisted corneal lenticule transplantation.
