中文摘要: 云南大姚由于环境青石棉污染和使用,成为世界上少有的恶性胸膜间皮瘤(MPM)高发区之一。该肿瘤发病人群特殊、病例及组织标本收集困难,其诊治成为医学界一大难题。我们的前期研究发现CT检查虽然对进展期MPM敏感性高,但早期MPM检出率不高。国外研究显示血清可溶性间皮素相关蛋白(SMRP)浓度升高可较敏感诊断MPM,但由于其它肿瘤也可导致其浓度升高,所以存在较高假阳性。本研究拟利用诱发大鼠MPM模型,行CT平扫加动态增强扫描及血清SMRP测定,利用血清SMRP较高敏感性,CT高空间分辨率准确确定胸膜病变及肿瘤特征,探讨联合CT与血清SMRP测定提高MPM早期诊断率及诊断准确性的价值,为临床建立一种更准确诊断MPM的新方法提供实验依据,同时对指导治疗及评估预后等也具有重要价值。

英文摘要: Da yao county in Yun Nan province is one of the few places with high incidence of malignant pleural mesothelioma(MPM) in the world, due to naturally environmental pollution by crocidolite asbestos and its use. Both diagnosis and treatment of MPM has long been challenging in medicine because this tumor involves specific population, its clinical cases and histological specimen are difficult to collect. Our previous study revealed that CT has relatively low sensitivity to early MPM although it is sensitive to advanced MPM. Foreign clinical studies showed that elevation of serum soluble mesothelin related protein(SMRP) is a sensitive marker for diagnosing MPM but with high false positive because SMRP elevation could also be found in other tumors. So this study plans to perform CT plain scan, contrast enhancement scan and SMRP blood test on induced MPM in rats, hoping that the CT high spatial resolution and its dynamic contrast enhancement features in morphology serving to localize pleural diseases and identify neoplastic features, in combination with the quantitative test of serum SMRP that has high sensitivity, could improve the lesion detection and the accuracy of MPM grading, which would provide an experimental basis for a new more accurate diagnostic method and could have important value in guiding therapy as well as prognosis evaluation.
