中文摘要: 皮层下血管性痴呆(SVD)具有一定的可治性、可逆性,早期诊治意义重大,但目前缺乏早期诊断的无创性定量指标。研究表明神经回路损伤是认知障碍的重要发病基础,而SVD发病存在多个脑区功能的相互协调和交互作用异常,提示全脑,特别是额叶皮层下区域"特定神经回路"的结构和功能异常参与了SVD发病过程。因此,本研究拟在广泛普及的MRI常规检查基础上,采用新型多模态MRI技术对其进行综合分析:⑴通过扩散张量成像(DTI)分析脑白质纤维的完整性,探讨SVD的脑结构网络病理异常模式;⑵通过静息状态功能磁共振(fMRI)探讨SVD的脑功能网络病理异常模式;⑶进行多模态磁共振数据联合分析,构建SVD的多模态脑网络模型,探讨其全脑结构、功能网络间的相互关系,进而研究它们在SVD各脑区相互作用时的动态变化关系。本研究不仅将增进对SVD发病机制的理解,并可望从系统和神经网络水平发现SVD早期诊断的无创性定量生物学标记。

英文摘要: Subcortical vascular dementia (SVD) is somehow treatable and revertable, and so its early diagnosis and treatment are of vital importance. But there's still no noninvasive and quantitative biomarks for its early diagnosis. Studies showed that damages to nerve circuitry is an important pathogenesis factor of cognitive impairment, and there are intercoordination and interaction abnormalities among multi brain districts in SVD.These suggest that abnormalities in structural and function of "special nerve circuitry" in whole brain, especially in subcortical area of frontal lobe, may be involved in the pathogenesis of SVD. So our study is to verify this hypothesis by comprehensive analysis of new multimode MRI data: i). Investigate the integrity of the white matter of SVD by DTI and establish its pathologic structural pattern of the brain network; ii). Explore the integrity of the brain activity of SVD by resting-state fMRI and establish its pathologic functional pattern of the brain network; iii). Combine analyze the above multimode MRI data and set up the model of multimode brain network of SVD, investigating the interaction between its whole brain structural and functional networks and their dynamic changes.
