中文摘要: 急性肾损伤(AKI)现有治疗手段肾脏替代治疗无法对其恢复起促进作用。由于活化免疫细胞在AKI的病理生理过程中起重要作用,针对其进行干预可能是治疗AKI的新途径。小鼠动物模型被证实可缓解AKI损伤的方法-化学物质清除某种免疫细胞的手段在临床无法应用。本研究拟通过大动物不同AKI模型,采用体外循环离心式选择性血细胞分离技术,来部分清除循环中单个核细胞、粒细胞或血小板,观察其对AKI损伤程度及恢复过程的影响,并进一步观察其效果可能与循环中循环活化免疫细胞水平降低的关系,是否影响AKI肾组织细胞浸润程度及损伤恢复过程,是否影响动物最终肾功能恢复及预后,特别是在合并脓毒症情况下是否会影响机体抗感染能力。如此试图为AKI的治疗寻找到一条新的途径。

英文摘要: Renal replacement therapy, a way to treat patients with acute kidney injury(AKI), can not facilitate the recovery of renal function. Regarding the fact that activated immune cells have been proved to play an important role in the pathogenesis of AKI, intervention therapy targeting on these cells may be a novel way to treat AKI. Depletion of specific immune cells using special agents, although being proved effective in amelioration of AKI in animal models, is not feasible for clinical application. This study is designed to investigate the effect of removal of mononuclear cells, neutrophils, or platelet from peripheral blood by extracorporeal blood cell separation technique(extracorporeal immune cell intervention therapy, EICIT) on treatment of AKI through multiple AKI models. Herein the effects on the severity of kidney injuries, the recovery of renal function, and the associations between the effects and the changes of circular activated immune cells counts will been investigated, as well as the inflammatory cell infiltration in kidney . Particularly, the potential deteriorative effect of removal of immune cells by EICIT in sepsis-associated AKI will be also studied.
