中文摘要: 同种异体肌腱移植重建前交叉韧带(ACL)存在排斥反应并影响疗效。本课题组首次发现异体肌腱移植后关节液Th17明显升高,并证实其与移植物松弛相关;进而发现Th17的主要效应因子IL-17A抑制膝关节腔内的滑膜间充质干细胞(SMSCs)的免疫调节能力。进一步研究发现IL-17A可能通过上调Bcl-2抑制SMSCs自噬,导致其免疫抑制功能异常。本研究拟以此为切入点,探讨IL-17A通过上调Bcl-2抑制Beclin1/Vps34/III 型PI3K自噬核心复合物的形成,从而影响SMSCs自噬启动的机制;在体外研究的基础上构建羊ACL重建动物模型,在体内深入探讨同种异体肌腱移植后IL-17A参与排斥反应的自噬相关机制,同时确定有效调节位点。本项目作为移植领域研究的一部分,有望为改善ACL 异体重建的临床疗效寻找新的突破,同时为促进移植领域的发展提供新的思路和有力补充,具有重要的研究意义和临床价值。

英文摘要: The rejection exists after allograft ACL reconstruction and affects patients' prognosis. Our group have reported for the first time that Th17 elevated dramatically in synovial fluid after transplantation and was related to graft relaxation. Our research also found that IL-17A, the main effector of Th17, may suppress immune regulation of SMSCs. Then our further research revealed that IL-17A could inhibit SMSCs autophagy through upregulation of Bcl-2.Based on this founding, this study will explore the mechanism that IL-17A inhibits the initiation of SMSCs autophagy through upregulating Bcl-2 which could bind to Beclin1/Vps34/ type-III PI3K core complex. We will also construct ACL reconstruction sheep model on the basis of in vitro studies and investigate the mechanism of IL-17A mediated SMSCs autophagy inhibition which could aggravate rejection in depth and determine the effective regulatory sites. As a part of transplant field, this research could improve the clinical efficacy of ACL allograft reconstruction and look for new breakthroughs, as well as promote the understanding of transplantation. For all above, our study has important significance and great clinical value.
