中文摘要: 近年来,剪切波超声弹性成像开始应用于乳腺病变的诊断。许多研究发现在剪切波弹性成像时,乳腺恶性病变周边区域可出现典型的高硬度现象。我们在之前的一项研究中,也发现了乳腺恶性肿瘤的这一特征,并将其命名为硬环征(stiff rim sign)。然而,目前尚没有针对硬环征形成机制的研究。因此,在本研究中,我们拟:①在体外使用弹性模量测量仪器,分别测量切除的乳腺病变内部区域和周边区域的真实弹性模量,并和剪切波弹性成像时在体内测得的相应部位的弹性模量相比较,探讨硬环征形成的生物力学机制;②通过特殊染色,在病理切片上对乳腺病变内部区域和周边区域胶原纤维含量进行半定量分析,并和体内剪切波弹性成像测得的相应部位的弹性模量进行相关分析,探讨硬环征形成的组织病理学基础。我们相信,本研究课题将深化我们对剪切波弹性成像组织病理学和生物力学基础的理解,使这一技术更好应用于临床。

英文摘要: More recently, shear wave elastography have been developed and used in breast lesion evaluation. Many studies found that some malignant breast lesions showed typical peri-tumoral stiffness in shear wave elastography. We, in a previous study, have named this finding the stiff rim sign, and found that the stiff rim sign yielded high sensitivity and specificity in the diagnosis of breast lesions. However, the cause of thestiff rim sign has not been studied. Thus, we aim to explore the histopathological and biomechanical basis of the stiff rim sign, by evaluating the relationship between the stiff rim sign and the content of collagen fibers in both the interior area and the peripheral area of breast lesions, and the relationship between the stiff rim sign and the elastic moduli of both the interior area and the peripheral area of breast lesions measured under compression in vitro. We believe that this basic research could help understand the histopathological and biomechanical basis of breast shear wave elastography.
