中文摘要: 对贵州省贵阳市汉族和黔南州布依族、苗族胃镜检查患者进行幽门螺杆菌的分离培养、鉴定,以琼脂稀释法检测各菌株药物敏感性,对各菌株进行耐药相关基因片段的PCR扩增测序和序列比对分析;以PCR的方法检测各菌株毒力基因或等位基因,进行毒力基因型或亚型的鉴别,以及对cagA3'端可变区扩增测序并翻译为氨基酸序列后进行 Glu-Pro-Ile-Tyr-Ala (EPIYA) 基序的比较和聚类分析。将本省汉族、布依族、苗族来源的幽门螺杆菌耐药表型和耐药基因突变特征、临床类型和毒力基因多态性进行比较和统计学分析,展示我省主要少数民族来源菌株的毒力和耐药相关基因结构特点,补充中国幽门螺杆菌菌株中少数民族来源菌株的分子流行病学特征,为进一步研究无创性筛查幽门螺杆菌致病菌株感染和耐药性快速检测,制定个性化治疗方案奠定基础。

英文摘要: This study is aimed to characterize the geographic diversity of Helicabacter pylori strains isolated from the Han people of Guiyang city and from the Buyi and the Miao people of Qiannan autonomous prefecture.The regional polymorphism of Helicobacter pylori strains will be evaluated in two aspects:1. Antibiotic resistance phenotype and related gene mutations.2.Distribution of clinical infection type including gastritis,gastric ulcer,duodenal ulcer and gastric cancer,and distinct diversity of virulence genes including cagA,vacA, iceA and HP0519 genes . Helicobacter pylori will be isolated from patients and antibiotic resistance will be detected by agar dilution method.The resistance assosiated gene fragments will be amplyfied by PCR and then sequenced.Gene sequences of susceptible strains and resistant strains from the Han ,the Miao and the Buyi people will be compared respectively in order to characterize the regional polymophism of gene mutations of local resistant isolates. The presence of the cagA gene,vacA,iceA and HP0519 alleles will be determined by PCR from Helicobacter pylori DNA. Carboxy-terminal variable region of cag A will be amplified by PCR and then sequenced. The nucleotide sequences will then be translated into aminoacid sequences,and the Glu-Pro-Ile-Tyr-Ala (EPIYA) motifs patterns will be analyzed and grouped.Antibiotic resistance phenotype,resistance associated gene mutation,distribution of different clinical infection type and virulence gene characters of these three ethnicity will be compared by Chi-Square test.
