中文摘要: 重复经颅磁刺激(rTMS)是治疗难治性抑郁症(TRD)的有效方法,但其对TRD患者的疗效常常差异很大,研究认为与rTMS治疗靶点的定位紧密相关,然而目前临床实践中仍然缺乏理想的个体化靶点定位方法。我们近期关于抑郁症患者情绪面孔功能磁共振(fMRI)研究发现,杏仁核与背外侧前额叶皮层之间存在显著负功能连接。本项目将基于任务态和静息态功能磁共振影像,采用激活、功能连接等分析方法,探索基于影像学的个体化TMS刺激靶点定位方法,并与传统定位方法比较,考察不同定位方法TMS治疗TRD的临床疗效差异。结合治疗前后临床量表的评估与fMRI脑功能活动分析,初步探索rTMS治疗TRD的神经机制

英文摘要: Repetitive transcranial magnetic stimulation (rTMS) is an effective method used in the treatment resistant depression (TRD). Therapeutic effectiveness of this method is variable among TRD patients, which is thought to be related to the location of target area in rTMS treatment. However, in clinical practice,there is still lack of the ideal location method for identifying individualized target. One of our recent functional magnetic resonance imaging(fMRI)investigation on depression patients revealed the significant functional connectivity between amygadala and dorsal lateral prefrontal cortex for the emotional face identifying task. Then, this project will employ task state and resting state fMRI and use the analysis methods, i.e. activation, functional connectivity, to explore individualized location methods in rTMS treatment. Further, we will compare the therapeutic effectiveness between the new and traditional location methods and explore the neural basis underlying the rTMS treatment on TRD patients through clinical assessments and fMRI measurements.
