中文摘要: 由于儿童言语测听难于操作,大多数儿童人工耳蜗(Cochlear Implant,CI)研究仅仅局限于简单基本的感知测试。而CI儿童相对较难的汉语普通话言语识别(如多说话者的双音节词和语句识别)的研究更是非常有限。此外,竞争性音乐识别对于CI患者来说尤其困难,音高,音色和时间因素可能用于竞争性旋律的分离和分组。本研究探究CI儿童的人口统计学因素对相对较难的汉语普通话言语识别表现的影响以及音高,音色和时间因素对CI患者的竞争性音乐识别表现的影响。本研究测试了37名CI儿童的多说话者(2男,2女)的开集汉语普通话双音节词和语句识别,同时测试了CI患者和听力正常者的带有和不带有掩蔽的音乐轮廓识别。结果显示早期人工耳蜗植入对重度耳聋儿童具有重要意义。对相对较难的言语识别测试的较好表现表明开集词汇和语句识别可用于年长的CI儿童的言语识别表现评估。听力正常人群竞争性音乐识别表现很好;而CI患者的表现较差。有些CI患者可以应用或综合音高,音色和时间因素进行竞争性音乐的识别,而多数则不能,表明较差的分离表现是由于人工耳蜗的较差的频谱分辨率。

英文摘要: Because of difficulties associated with pediatric speech testing, most pediatric cochlear implant (CI) speech studies necessarily involve basic and simple perceptual tasks. There are relatively few studies regarding Mandarin-speaking pediatric CI users' perception of more difficult speech materials (e.g., words and sentences produced by multiple talkers).Furthermore,the competing music recognition is especially difficult for CI users. Pitch, timbre, and/or timing cues may be used to stream and segregate competing musical melodies.The present study investigated how pediatric CI patient demographics influence speech recognition performance with relatively difficult test materials and methods and the effect of pitch, timbre and timing on the performance of competing music recognition for CI users. In this study, open-set recognition of multi-talker (two males and two females) Mandarin Chinese disyllables and sentences were measured in 37 Mandarin-speaking pediatric CI users and melodic contour identification was measured in CI and normal-hearing (NH) listeners, with and without a competing masker.The result showed early implantation provided a significant advantage for profoundly deaf children. Performance was generally quite good for the relatively difficult materials and tasks, suggesting that open-set word and sentence recognition may be useful in evaluating speech performance with older pediatric CI users. NH performance was near-perfect across different conditions. CI performance was significantly poorer than that of NH listeners.While some CI subjects were able to use or combine timing, pitch and/or timbre cues, most were not, reflecting poor segregation due to poor spectral resolution.
