中文摘要: 体外循环(CPB)是心内直视手术的一种重要辅助手段,CPB术后心肺功能障碍仍是最常见而有待于解决的关键问题。MAPK通路及细胞凋亡是体外循环心肺影响的重要病理途径。通过前期肺切除针刺(复合)麻醉规范化方案的研究,我们证实了针麻具有脏腑保护作用。基于上述临床研究基础,围绕MAPK通路及细胞凋亡之关系,本课题①通过持续监测心电图、血压、鼻咽温度、肛温及检测动脉血气,观察针刺对大鼠外循环模型的作用效应;②系统观察体外循环后心肺p38MAPK、JNK(Bim)、ERK蛋白激酶的动态活化,为进一步研究心肺保护机制提供理论基础;③观察MAPK信号通路在体外循环后变化及细胞凋亡的影响,探讨针刺在体外循环后心肺损伤中的保护作用及机制。本研究将揭示针刺麻醉的"针刺-脏器保护相关效应"机制,从而有望突破以往针刺麻醉机制研究的"瓶颈",为针麻基础研究寻求镇痛效应以外的新思路。

英文摘要: Cardiopulmonary bypass (CPB)is an important adjunct to the open heart surgery.However, postoperative cardiopulmonary dysfunction remains as the most common and the key issues to be solved. It is demonstrated that the MAPK pathway and apoptosis effects are the main cardiopulmonary pathological way of cardiopulmonary bypass. From previous studies on standardized program of acupuncture anesthesia, we confirmed that acupuncture anesthesia has effects of organ protection. Based on the above clinical research and the relationships of the MAPK pathway and apoptosis, the aims of the study are ①Through continuous monitoring of ECG, blood pressure, nasopharyngeal temperature, rectal temperature and arterial blood gas, observe acupuncture effect on the rat outer loop model; ②Systematically observe post cardiopulmonary bypass cardiopulmonary p38MAPK, JNK(Bim)and ERK protein kinase dynamic activation, and provide a theoretical basis for the further study of cardiopulmonary protection mechanisms; ③ observe the changes of MAPK signaling pathway and apoptosis after cardiopulmonary bypass to explore acupuncture in vitro Protective effects and mechanisms in the circulation after cardiopulmonary injury.
