中文摘要: 伴中央颞区棘波良性儿童癫痫是最常见的小儿原发性癫痫类型之一。以往多认为由于其发作和脑电图异常可能自然消失、远期预后好,近年来神经心理研究发现患该病的儿童与健康同龄儿童相比可能有着学习记忆等方面的问题,但对其发生机制及药物治疗的必要性和时机尚存争议。本课题拟采用以同步脑电联合功能磁共振成像为主的神经影像技术,针对BECT患儿诊治过程中不同阶段(治疗前后)EEG和fMRI中的尖、棘波和注意网络情况进行观察,全面地评价癫痫患者的注意功能。本研究将有助于揭示伴中央颞区棘波良性儿童癫痫注意网络变化状况及其可能机制,寻找癫痫注意功能损伤的模式及其相关的临床因素,也有助评价抗癫痫药物对注意网络的影响,为指导临床用药提供一个灵敏的生物信号特征和理论依据。

英文摘要: Benign Epilepsy of Childhood with Centrotemporal Spikes(BECT)is the most common idiopathic and also the most common type of epilepsy in childhood. Traditionally, it is considered that children with BECT have no cognitive impairment. However, some recent studies show BECT patients have more difficulties in learning and memory compared to age-matched healthy children. EEG-fMRI was used to reveal the deactivation phenomenon of the attention network in the patients with BECT epilepsy, and can be used to reflect the normal or abnormal state of specific corresponding functions. It is likely that learning of attention impairment and the physiopathologic mechanism of BECT is helpful for finding the identification of attention impairment and the crosstalk with the clinic factors. Therefore we can supply a sensitive biomarker for the early intervention with medication of epilepsy by using functional magnetic resonance imaging (fMRI).
