中文摘要: 硬脑膜动静脉畸形(dural arteriovenous malformation,DAVM)的脑静脉返流(cerebral venous reflux ,CVR)是决定临床症状的重要因素,是决定是否进行干预及选择治疗方案的重要依据,也是判定治疗是否成功的重要指标之一。但是CVR影响DAVM症状、治疗后转归的血流动力学机制尚不明确。本课题将基于患者3D-DSA 图像得到个体化的DAVM三维几何模型,进而建立具有病人特异性的伴有CVR的DAVM生物计算流体力学(CFD)模型并对其血液流动进行数值模拟。通过对栓塞前后局部畸形区域的血流动力学状态进行分析,探讨脑静脉返流型DAVM自然病史进展、颅内出血风险以及栓塞治疗预后与血液动力学参数间的定量关系,并通过分析畸形局部血流量、血流速度、压力等参数比较各种栓塞方案的优劣。课题有助于进一步丰富我们对这一疾病的认识,为临床治疗提供个体化和量化的依据。

英文摘要: The presence of cerebral venous reflux(CVR)in dural arteriovenous malformations(DAVMs) is an aggressive feature that influences clinical symptoms and the therapeutic selection,and also an important factor to predict the prognosis after embolization.However,the hemodynamic mechanism that CVR influences the clinical presentations and prognosis after embolization is not clear.The study will construct patient-specific biofluid mechanics models based on patients' 3D-DSA image and perform computational fluid dynamic(CFD) analysis. By analyzing the hemodynamic characteristics at the abnormal shunts of DAVMs with CVR before and after embolization, we will investigate the quantitative relationships between the local hemodynamic parameters and the natural history, risk of intracranial hemorrhage and the prognosis after embolization. Thus we can develop more reasonable therapeutic schedules. The study will be helpful for us to further understand DAVMs, and provide individual and quantitative basis for treatment of this disease.
