中文摘要: 量化缺血性脑组织超急性期的损伤程度包括微观组织结构完整性、形态及血脑屏障是组织窗评价的基础,也是选择治疗措施的前提。如何实现无创、动态、量化这些变化仍是一个未解决的问题,难点在于缺乏综合性技术平台及合适的观察指标。能不能以缺血后脑水肿的分布及演变为线索,评价不同时段受损脑组织的变化及血脑屏障损伤?本研究在结合前期基础上发现磁共振成像技术可以区分细胞内水肿及血管源性水肿并在指导治疗中取得疗效。本项目拟采用MCAO大鼠动物模型为研究对象,研究不同缺血时段脑组织细胞毒性水肿与血管源性水肿的演变及量化,同时包括神经元完整性、血脑屏障损伤。以磁共振成像包括DWI-MRS、DKI、SWI所得量化指标为平台,分别评价细胞内含水量的变化、神经元完整性、含铁血红蛋白的渗出,同时以不同病理检查验证结果,构建评价缺血后的组织窗影像学平台,探索缺血损伤后水肿的分布及演变机制,为临床应用奠定基础。

英文摘要: To quantify the damage degree of hyperacute brain tissue including integrity, microstructure, morphology and blood-brain barrier is not only the basis evaluation of tissue window, but also the premise of the choice for therapeutic measures.How to quantify these changes noninvasively and dynamically is still an unsolved problem,the difficulty lies in the lack of a comprehensive technology platform and appropriate observation index. Can the evolution of ischemic brain edema as a clue to evaluate the change of the damaged brain tissue in different periods and the lesion of blood-brain barrier? The study found that magnetic resonance imaging on the basis of previous techniques can distinguish between intracellular edema and vasogenic edema, thus obtaining curative effect in guiding treatment. This study use the animal model of MCAO rats for research, and detect the evolution and quantified degree of the cytotoxic edema and vasogenic edema in the different period of ischemia brain tissue, including the neuronal integrity and the lesion of blood brain barrier. Using the quantitative indicators from the magnetic resonance imaging technique such as DWI-MRS, DKI, SWI as a platform,respectively evaluate the changes in intracellular water content, neuronal integrity and iron hemoglobin leakage. At the same time, we validate the results through the different pathologic examination and construct the imaging platform which is used to evaluate the ischemic tissue window, then explore the distribution and evolution mechanism of edema after ischemic injury, ultimately, lay the foundation for clinical application.
