中文摘要： 2011年底，我国政府举办的基层医疗机构已全部实施了基本药物制度，但基本药物制度实施效果的监测和评价工作尚没有建立。本研究拟在完善国家基本药物制度实施效果评价方法的基础上，对我国国家基本药物制度实施效果进行评价，探寻基本药物制度实施过程中出现的问题和影响基本药物制度实施效果的因素。研究在二手资料分析的基础上，确定适合中国国情的WHO/HAI标准调查法、药品灾难性支出影响和致贫作用评价，对基本药物的可及性进行评价；借鉴WHO选择性用药指标，从医疗机构和处方两个方面对基本药物的合理使用进行评价；实地调查收集基层医疗机构类型、门急诊量、医院收入构成、药物配备、用量、处方和患者基本信息等数据，采用多水平回归方法探寻影响基本药物制度实施的关键因素，为我国国家基本药物制度监测和评价工作奠定理论和实证基础，使我国国国家基本药物制度日趋完善。

英文摘要： China has established the National Essential Medicine Policy(NEMP) in all public primary health care institution by 2011. But the effect of NEMP hasn't been evaluated yet. This study has 3 parts: (1)to improve an evaluation method for China's NEMP, (2) to analyse the implementation outcomes of the NEMP based on the evaluation method, (3) to identify the key factors that affect the implementation outcomes. This study involves a literature review and field studies. The data was analyzed qualitatively and quantitatively. The qualitative analysis led to the development of the NEMP evaluation method, such as WHO/HAI standerdized approach and Selected Drug Use Indicators.The study will collect Data about the accessibility of essential medicines and the implementation outcomes of the NEMP from primary medical institution and private drugstores. The quantitative methods included the WHO/HAI standard investigation approach, the catastrophic method, the impoverishment method, comparison analysis, multiple linear regression, and so on. Finally, this study will provide detailed suggestions for policy-makers to improve the NEMP.
