中文摘要: 流行病学及动物实验研究均表明慢性牙周炎与慢性阻塞性肺疾病(COPD)存在紧密的相关性,但其关联的具体机制不清且缺乏相关研究。微生物在慢性牙周炎以及COPD的发生发展中均起着重要作用。口腔与呼吸道相通,研究表明慢性牙周炎患者肺部可检出牙周致病菌,因此我们提出假设:口腔微生物群落在慢性牙周炎与COPD的相关性机制中起着重要的作用。我们将通过16S rRNA高通量基因测序、宏基组等方法检测并比较COPD伴慢性牙周炎患者在稳定期与急性发作期口腔与肺部微生物群落的一致性以及相关微生物的毒力因子、代谢途径等特点;比较COPD患者急性发作期治疗前后口腔与肺部微生物群落的变化;探讨口腔微生物群落在慢性牙周炎与COPD相关性机制中的作用。本研究将首次从微生物群落角度揭示慢性牙周炎与COPD的关联机制,并为通过牙周干预进行COPD的辅助防治提供依据。

英文摘要: A strong association between chronic periodontitis and chronic obstructive pulmonary disease (COPD)has been suggested by a number of recent epidemiological and animal studies. However, the biological mechanisms underlying this association remain unclear and rarely been studied. Microbe plays an important role in the development of chronic periodontitis and COPD. Oral cavity connects with respiratory tract, and it has been demonstrated that periodontal pathogens could be detected in the lower respiratory tract. We thus hypothesize that oral microbe plays an important role in the underlying mechanisms between chronic periodontitis and COPD. Through 16S rRNA sequence analysis and metagenomics,we propose to detect and compare the microbe concordance between the oral cavity and lower respiratory tract in stable and acute exacerbation period of COPD patients with chronic peridontitis, and the virulence factors and metabolic pathways of the associated microorganisms. The microbe alterations in the oral cavity and lower respiratory tract before and after treatments of COPD acute exacerbation will also be detected and compared. This study will be the first to reveal the biological mechanisms underlying the association between chronic periodontitis and COPD, and provide basis for therapeutic potential of periodontal interventions for the prevention and treatment of COPD.
