中文摘要: 慢性肾脏病(chronic kidney disease,CKD)已经成为重要的公共卫生问题,最终将进展为终末期肾病(End stage renal disease,ESRD)。进行性肾脏纤维化(renal fibrosis,RF)是CKD进展为ESRD的关键。目前RF诊断主要依赖肾活检,由于其创伤性检查的本质,无法对患者实施动态监测。最新研究显示,众多miRNA参与了RF这一复杂的病理学过程,而尿液miRNA能够反映肾脏病的进展,因而可能成为CKD无创、新型诊断标志物。本研究拟在既往RF发生机制和尿液基因生物标志物研究基础上,应用miRNA芯片对CKD患者尿液miRNA进行初步筛查,并利用实时荧光定量PCR技术对候选生物标志物进行系统验证,对有意义的miRNA结合临床病理学关联分析,探讨肾脏纤维化新型无创的生物标志物,为临床RF诊断和动态监测提供重要的理论和实验依据。

英文摘要: Chronic kidney disease (CKD) has become a public health problem worldwide, which will ultimately lead to end stage renal disease (ESRD). Irrespective of the initial causes, renal fibrosis is the common pathway of ESRD. Currently, renal pathological examination is the gold standard for evaluating RF in CKD. However, due to the invasive nature of the procedure and the associated potential complications, it is impractical in routine clinical practice for repeated renal biopsies to be used to monitor the development of RF. Recent studies have demonstrated that multiple miRNAs take parts in the complex pathological process of RF, and urinary miRNA profile could reflect progression of CKD. Thus, screening miRNAs in urine may provide important information for RF monitor. This study aims to screen urinary miRNA levels in various stages of CKD by miRNA microarray and real-time PCR technique, and systemic validate the potential biomarkers by clinic-pathology study. These studies would provide novel and non-invasive biomarkers for RF and important theoretical/experimental basis for the diagnosis and dynamic monitoring of renal fibrosis.
