中文摘要: 大气细颗粒物(PM2.5)如何引发慢阻肺的急性加重,最终导致患者急诊率、入院率和死亡率的增加一直不很清楚。本研究拟在前期工作的基础上,采用定组研究设计,追踪观察同一组慢阻肺患者在经历实际生活环境中大气PM2.5污染水平从低—高—低的变化的三个不同时期,探讨其PM2.5暴露水平、呼吸和心血管功能改变、机体氧化应激和炎症水平以及慢阻肺急性加重之间的关系;结合M2.5化学组分分析结果,解析并量化不同来源大气PM2.5对慢阻肺患者预后的短期影响及其与急性加重的关系;进一步采用高通量代谢组学方法,比较慢阻肺患者和健康人群大气PM2.5暴露后的呼出气冷凝液和血清的代谢物组差异,发现异常代谢通路,提出敏感效应标志物。以期初步解析不同来源大气PM2.5暴露所致慢阻肺急性加重的潜在作用模式。研究结果将为我国政府采取有针对性的措施,改善空气质量,保障易感人群健康提供科学数据。

英文摘要: It has been reported that ambient fine particulate matter (PM2.5) exposure may induce exacerbation of chronic Obstructive Pulmonary Disease(COPD), increase the rates of emergency, hospitalization and mortality in COPD patients, however, the potential mechanism remains unclear. Based on our previous work, this study was designed as a panel study, follow-up a group of COPD patients who experience a real-environment of PM2.5 levels change from low - high - low in three different periods, investigate the correlations between their PM2.5 exposure levels, respiratory and cardiovascular function changes, oxidative stress and inflammation levels, and exacerbation of COPD; Combined with PM2.5 chemical constituents, to recognize and quantify the correlation between short-term effects on COPD patients induced by source-appointed PM2.5 and exacerbations of COPD; And further with the support of metabolomics, to compare the differences of metabolomics in induced exhaled breath condensate and serum between COPD patients and healthy people exposed to PM2.5, find abnormal metabolic pathways, put forward sensitive effect markers, and clarify the potential role of PM2.5 in exacerbation of COPD. The results will be helpful for our government to control air pollution, improve air quality, and protect the health of vulnerable populations in China.
