中文摘要: 前颞叶切除术(ATL)术后部分病人受损的记忆功能会随时间得到一定程度的改善,研究这种记忆功能重塑和补偿的内在机制有助于预防ATL术后记忆障碍的发生和指导记忆康复治疗,对探索和揭示大脑的记忆和脑功能的可塑性具有重要意义。但是,目前这类研究尚缺乏有效的量化评估研究手段。本项目拟结合功能核磁共振(fMRI)和虚拟现实技术,利用虚拟迷宫模拟的陈述性和程序性记忆任务情景作为刺激,选择ATL病人术前及术后不同时间阶段的观察窗,观察ATL术前术后在迷宫任务作业时脑激活区的动态变化,特别是对侧颞叶的脑激活区变化;精确定位记忆功能重塑和补偿区,再利用核磁共振弥散张量成像(DTI)及DSI观察记忆功能重塑和补偿区与其它记忆区的纤维联系,发现重塑的记忆网络,深入探究ATL术后记忆功能的重塑和补偿机制;藉此建立一套ATL术前记忆功能的fMRI标准化预测评估系统,为预防ATL术后的记忆障碍提供指导作用。

英文摘要: Some patients’ impaired memory after Anterior temporal lobectomy (ATL) may improve gradually as time passes by. Therefore, to study the mechanism of memory function replasticity and compensation will prevent the occurrence of memory impairment and provide an effective guidance for memory rehabilitation for patients after ATL. It is of great significance to explore and reveal the plasticity of the brain''s memory and other functions. However, an effective quantitative evaluation system has not been established yet. In herein project, we will adopt functional magnetic resonance imaging (fMRI) and virtual reality technology to observe the dynamic changes of patients’ brain activation areas during the operation of the maze task before and after ATL, especially the changes occur at the opposite temporal lobe. Declarative memory task scenarios and procedural memory task scenarios produced by virtual maze will be used as stimulus and different timing of the ATL patients will be taken as the observation windows. Then the area of memory replsticity and compensation will be présed located. Under nuclear magnetic resonance diffusion tensor imaging (DTI), the fiber connections of the memory replasticity and compensation area with other memory areas will be observed to detect a new memory network.
