中文摘要: 儿童失神癫痫(CAE)是一种常见的儿童期癫痫。研究表明:高频振荡(HFOs)在癫痫的发病中可能起重要作用。我们前期对10例CAE患儿HFOs的初步研究结果发现:在所有CAE患儿的大脑中均存在HFOs,HFOs主要定位在内侧前额叶,HFOs出现时间与失神发作频率有关(HFOs出现时间越长,失神发作越频繁)。为明确HFOs对于CAE能否成为潜在、新的生物标志,本项目利用时频分析(实时频谱图及累积频谱图)及动态磁源成像,在频谱(频率、时间)及磁源(空间、能量)水平上,探究CAE患儿和年龄相匹配的健康对照者脑内HFOs的区别、CAE患儿服药前后HFOs的变化、HFOs与发作期放电时间/失神发作频率之间关系、CAE患儿HFOs由发作间期、发作前期至发作期的演变、CAE患儿HFOs和常见的CAE神经心理问题(如注意力缺陷)关系。本研究的开展将更好的管理CAE患儿并改善其整体生活质量。

英文摘要: Childhood absence epilepsy (CAE) is a common childhood-onset syndrome. Studies suggested a key role of high frequency oscillations (HFOs) in initializing seizures. High frequency magnetoencephalography(MEG)data of 10 children with CAE have presented that HFOs were predominantly localized in the medial prefrontal cortex, and the rate of fast ripples was associated with seizure frequency. In order to determine the possibility of HFOs taken as biomarkers for study of CAE, in this study the time-frequency analysis and dynamic magnetic source imaging will be introduced to examine and compare HFOs in children with CAE and age-matched healthy controls, and assess the changes of HFOs in children with CAE before and after AED treatment, and depict correlation between seizure frequency and HFOs at spectrogram (frequency and time) and magnetic source (space and power) levels. In addition, the change of HFOs during the transition from interictal, preictal to ictal stages and the correlation between HFOs and neuropsychological deficits will be clarified. This study will contribute to a better clinical management and improvement in the overall quality of life for children with CAE.
