中文摘要: 单纯餐后高血糖型糖尿病(Isolated Post-challenge Hyperglycaemia, IPH)是糖尿病的一个亚型,主要表现空腹血糖(FPG)低于7.0 mmol/L,而2h餐后血糖(2h-PG)大于或等于11.1mmol/L。流行病研究表明约40-50%的新诊断糖尿病患者属于IPH患者,因此,对IPH患者早期诊断尤为重要。本课题将代谢组学技术应用于人群研究,首先考查不同膳食对不同人群的血清游离脂肪酸谱和生物标志物的影响;其次对不同人群血清游离脂肪酸谱、血清代谢指纹图谱和氢核磁共振谱进行比较研究,分析不同人群的血清代谢谱的不同之处;同时,对IPH患者进行跟踪试验,考察IPH患者发展成为混合型糖尿病(FPG ≥7.0 mmol/L,2h-PG≥11.1mmol/L)的时间点和发生率,并进一步研究其生物标志物的变化情况。这对早期预防和诊断IPH患者具有重要的理论和实际意义。

英文摘要: Isolated post-challenge hyperglycaemia (IPH),which is characterized by fasting plasma glucose (FPG) < 7.0 mmol/L and 2 h postprandial plasma glucose (2h-PG) ≥ 11.1 mmol/L, is a subgroup of diabetes. Epidemiological study indicated that 40-50% patients with 2h-PG ≥11.1 mmol/L in newly diagnosed diabetes had a FPG < 7.0 mmol/L. Therefore, it is important for diagnosing IPH patients as early as possible. In this study, we performed serum metabolite profiles from the same group of IPH, type 2 diabetes mellitus patients (T2DM), impaired glucose tolerance patients and controls using metabonomics method coupled with a multivariate statistics technique. Firstly, we investigate the effect of different diets on free fatty acid profile and biomarkers. Secondly, free fatty acids profiles, metabolite fingerprint and 1HNMR are detected with metabonomics technique in the subjects and biomarkers distinguished IPH patients from other groups are identified. Furthermore, IPH patients are followed up for two years and the point-in-time and incidence where IPH patients will be changed into T2DM patients (FPG>≥7.0 mmol/L and 2h-PG ≥11.1 mmol/L) are investigated.
