中文摘要: 中风是临床上致残率最高的疾病,研究中风后功能康复具有重大的科学意义和临床价值。针刺促进中风功能康复的临床疗效已得到认可,但还缺乏可靠的评价标准,其在介入时机的选择以及整体功能层次上的作用机制也有待进一步阐明。近年来脑功能成像研究提示中风后功能恢复程度与神经系统功能重组状态以及血管新生关系密切。基于一种新改进的mini-stroke大鼠中风模型,本项目拟采用高时空分辨的内源信号光学成像与激光散斑成像方法,来实现长时程(30天)纵向监测躯体感受皮层区域缺血再灌注后的功能重组模式以及梗死灶周围的血管新生过程。并进一步研究不同介入时间点(中风急性期和恢复期)的电针干预手段对这种缺血再灌注后功能重组与血管新生过程的影响。本研究有助于揭示功能重组、血管新生与行为功能恢复水平之间的内在规律,阐明针刺促进中风后功能康复的作用途径,为评价和优化不同针刺治疗中风康复手段,提高临床疗效提供新思路和新方法。

英文摘要: Stroke is the leading cause of adult disability in clinic. Understanding of the mechanisms underlying functional recovery in stroke is essential for basic and clinical studies. Acupuncture is a well-accepted clinical treatment of brain functional recovery, although its reliable evaluation criterion?is lacking.?Moreover, the intervening timing and the treating mechanisms of acupuncture in level of integrative brain function are still poorly understood. Recently brain functional imaging studies suggested that a tightly relationship between recovering outcome after stroke, functional reorganization and angiogenesis around infarct. Adopting a new rat mini-stroke model, intrinsic signal optical imaging and laser speckle imaging with high spatiotemporal resolution are used here for long-term tracing the functional reorganization pattern and angiogenesis processing around infarct in rat somatosensory cortex suffering from focal ischemia-reperfusion. Furthermore, the influence of electrical acupuncture at different intervening time (acute phase and chronic phase) on functional reorganization and angiogenesis after stroke will be also investigated. This study will help to discover the relationship between recovering outcome after stroke, functional reorganization and angiogenesis around infarct, as well as to clarify the therapeutic effectness of acupuncture on functional recovery in stoke, suggesting a new way or method to evaluate and to optimize different acupuncture treatments of functional recovery in stroke?
