中文摘要: 胶质瘤的分子表达异质性明显,手术标本分子病理学研究是热点,而针对肿瘤全切困难、残余肿瘤决定预后等现实,仍缺乏分子影像学在体显示肿瘤异质性。胶质瘤细胞及新生血管内皮表达整合素受体αvβ3,我们前期发现其新型68Ga-PRGD2 PET/CT分子显像可准确显示肿瘤恶性级别,据此提出该分子显像能通过整合素受体表达和新生血管生成角度在体显示肿瘤间与肿瘤内的异质性、评估治疗效果。拟进一步深入研究,术前在体显示胶质瘤及新生血管的整合素受体,术中在显像及MRI增强融合导航下多点留取标本,实验证实其识别肿瘤恶性级别及新生血管的可行性,并通过与传统影像差异化区域的分子本质研究,从整合素受体分子表达层面在体揭示肿瘤异质性,术后随访并探讨该分子显像早期预测抗血管生成效果的优势。该研究增加胶质瘤整合素受体表达的理论认识,在分子表达层面判断肿瘤异质性,术后在体显示残余整合素,早期预测综合治疗尤其是靶向精准治疗效果。

英文摘要: Glioma is the most aggressive brain tumor type and a heterogeneous disease. In the advent of personalized medicine the understanding of intra-tumoural heterogeneity at celluar and imaging level is mandatory to improve clinical outcome. There has been so many molecular pathologies research recently. However, they just tell us the information about the resected portion of tumor. The following treatment result depends more on the residual portion of tumor. There is rare glioma molecular imaging research which could inform us the more totally characteristics of tumor in vivo and predict the treatment response or prognosis. Molecular imaging of integrin alpha(v)beta(3) expression in malignant glioma patients by 68Galacto-PRGD2 positron emission tomography (PET/CT) may allow for noninvasive determination of biological tumor characteristics and new vessel formation. We would investigate the molecular pathologies and integrin receptor expression and neovasculature on the patient’s integrin receptor imaging. We further follow up the patients with MRI, integrin 68Galacto-PRGD2 and 18F-FDG PET/CT, and conclude which one could predict more accurately about the patient treatment response and prognosis. In all, we predict that this research could noninvasively inform us the level of alpha(v)beta(3) expression in glioma patients, give us a new tool to show the tumor biology in vivo and predict the integrin receptor inhibitor treatment response and prognosis. It would meet the need for a more personalized and precise medicine in the modern treatment concepts for cancer patients.
