中文摘要: 神经影像学的进步提高了轻度创伤性脑损伤(mTBI)检出的精确度。然如何全面、整体评估mTBI患者检出的冰山一角下弥散性微结构损害和解释患者功能改变仍是临床面临的一个难题。人脑是高度复杂的时空动力系统,数据驱动的神经影像分析技术,有利于从磁共振数据的海量信息中进一步挖掘结构和功能大尺度网络的连接规律,是进一步探索和理解mTBI患者损伤后大脑完整系统改变及其恢复/重塑(演化)规律的最佳方法。本研究拟采集80-100例创伤后(0-9天、6和12月)mTBI患者的多模态MRI(包括vMRI、DTI、ASL和rs-fMRI)数据,数据驱动的大尺度复杂网络技术用于探讨mTBI患者创伤后的形态、结构和功能网络的空间拓扑属性改变特点,寻找mTBI可信的预后影像学标志,并结合临床功能评估探讨患者创伤后中枢结构重塑与功能恢复的可能机制。本研究的意义在于:从新的视角为理解mTBI患者的病理生理机制提供依据。

英文摘要: The advancement of neuroimaging has added increasing accuracy to the detection of the patients of mild traumatic brain injury (mTBI).However, it is still a challenge for clinicians when confronted with mining below the tip of the iceberg to comprehensively and integrally evaluate diffuse microstructure damage in mTBI patients, as well as to elucidate the functional changes. Human brain is a highly complex spatial and temporal dynamic system. Using neruoimaging data driven analysis techniques makes for mining the connection rules of structural and functional large-scale networks from huge amounts of information of magnetic resonance imaging. It is the optimal method to further explore and understand the alterations of complete brain system together with its recovery, reorganization or evolvement rule of mTBI.The present study intends to collect multi-modal MRI (including vMRI, DTI, ASL and rs-fMRI) data from 80-100 cases of post-traumatic (0-9 days, 6 and 12 months) patients with mTBI. Data driven techniques of the large-scale complex networks are applied to investigate the changes of morphological structures and spatial topological property of functional networks in mTBI patients to seek credible prognostic imaging markers for mTBI, and to investigate the plausible mechanisms of structural reorganization and functional recovery of nerve centers with clinical function assessment of mTBI patients.The point of this study is to provide a basis for understanding the pathophysiologic mechanism of mTBI patients from a new perspective.
