中文摘要: 实时跟踪应用于调强放疗最大的障碍是“标记点被叶片遮挡所导致的跟踪效果降低”。现有的解决方法为:1.利用适形野跟踪:适用范围小;2.利用kV级X射线或其他设备跟踪:增加额外剂量和额外设备。针对上述局限性,较为理想的方法是直接优化标记点可见性,即减少出束过程中标记点被叶片遮挡的概率。申请人的前期研究已建立了初步的可见性优化算法,但还不够成熟,体现在:1.必须保持剂量分布不变,导致优化搜索区域过小;2.只能优化“一个标记点可见的概率”。为此,本项目拟通过编程实现新的可见性优化算法。与现有算法相比,新算法:1.仅要求剂量分布满足临床要求,优化搜索区域扩大;2.可以优化“两个标记点同时可见的概率”。另外,本项目拟建立可见性优化策略,在穿刺植入标记点阶段就考虑可见性问题,优化植入位置/区域,以进一步提高可见性。因此,本项目的研究成果可以比现有方法更好的提高标记点可见性,为今后跟踪放疗的开展提供理论基础

英文摘要: For intensity-modulated radiation therapy (IMRT), the blockage of multi-leaf collimator (MLC) obstructs the application of real-time tracking radiotherapy. Current methods includes: a) tracking with conformal fields, but only for the simple tumor; b) using kV-X ray or other auxiliary imaging devices, but not good for the extra dose and extra device. For the above limitations, the better solution is to improve the marker visibility directly for IMRT, that is, to reduce the probability of MLC blockage during beam-on time. So, the aim of this study is developing optimization strategies and algorithms to improve marker visibility. The proposed strategies and new algorithms include three advantages: a) considering marker visibility from implantation; b) optimize marker visibility under the premise of satisfying clinical requirements; c) optimize the probability of at least two markers visible. Consequently, this study will improve the marker visibility during IMRT delivering, and provide theoretical fundamentals for future studies of real-time tracking radiotherapy.
