中文摘要: 由神经源性膀胱诱发的泌尿系统并发症是脊髓损伤(SCI)患者死亡的主要原因,寻找有效膀胱功能康复手段是临床急需解决的重要课题。骶神经电调节(SNM)干预现有的神经反射通路,恢复神经通路间兴奋和抑制的正常平衡关系,它对神经系统无损伤,保留了患者最终借助于神经再生恢复功能的希望,是对神经源性膀胱功能恢复与重建的一种较新且很有发展前景的方法。目前临床实践表明SNM对SCI膀胱功能障碍治疗效果不佳。研究者们推测对SNM的刺激模式进行优化并在SCI早期实施治疗将会改善治疗效果。本项目拟进行由刺激频率和刺激时间所决定的SNM刺激模式在早期完全性SCI大鼠膀胱功能障碍重建效果的研究,同时利用小动物PET研究大鼠脑血流量,探索高级中枢神经系统在SNM进行早期SCI膀胱功能障碍康复中的神经调节作用。本项目为SNM临床应用提供理论参考,为电调节在中枢神经系统损伤后早期功能重塑中的作用提供理论基础和科学依据

英文摘要: Urological complications following neurogenic bladder are main causes of death for patients with spinal cord injury (SCI). Efficient restoration of bladder function is main direction and urgent issue in clinic rehabilitation.Sacral neuromodulation (SNM), without damage to the nerve system, help to reestablish normal neural balance through enhancing or suppressing activity of the nervous system for the treatment of disease. Therefore, SNM is a promising therapy for neurogenic bladder.In clinic practice, SNM have little effect for bladder dysfunction following SCI. It is predicted that applying neuromodulation technologies on the SCI in early time, combining with the optimized stimulation parameters, may improve the efficiency.In this basic research, SNM will be used to treat bladder dysfunction following SCI in the erly time, in which the simulation mode is determined by stimulation frequency and stimulation time. Moreover, micro-positron emission tomography (PET) will be used to detect rat cerebral blood flow (CBF) for investigating the brain activity, determining how the SNM acts on high-level nerve centers involved in modulation of bladder function.If this project could be implemented, it will provide theoretical evidence for the application of SNM with treatment of SCI, and give scientific knowledge to unders
