中文摘要: 精神分裂症心理理论障碍是认知神经精神医学的热点问题,但其脑网络神经机制尚不清楚。基于正常成人被试的研究提示,理解他人心理的认知加工和应用他人心理的认知加工分别依赖于前额叶背内侧面与颞顶联合区的后向连接和前向连接功能。基于前期的研究结果,本研究提出精神分裂症心理理论损害的脑网络有效连接假设:精神分裂症的额-颞叶脑网络有效连接障碍是其心理理论认知损害的神经基础。我们通过基于计算机的指令交流任务,采用功能磁共振和动态因果模型的研究方法,研究精神分裂症患者在心理理论理解加工阶段和应用加工阶段的脑网络有效改变,验证精神分裂症心理理论损害的脑网络有效连接假设。并利用社会认知交互训练进行干预,从脑网络有效连接水平证实其对心理理论损害的治疗作用,间接验证精神分裂症心理理论损害的脑网络有效连接假设。研究有利于加深对精神分裂症心理理论障碍神经生物学基础的理解,为社会认知交互训练临床应用提供理论依据。

英文摘要: In the past more than 20 years, theory of mind deficits in schizophrenia has become an extremely active research field in Cognitive Neuropsychiatry. To date, however, the exact neural mechanism of brain network underlying impaired theory of mind in schizophrenia is not clear. Previous fMRI studies with normal adults indicated that processing others' mental states modulate the backward connections from the dorsal medial prefrontal cortex (dMPFC) to the bilateral temporoparietal cortex (TPJ), whereas applying others' mental states modulate the forward connections from the TPJ to dMPFC. Based on previous research, we hypothesize that impaired theory of mind in schizophrenia results from a disruption in fronto-temporal effective connectivity. In this study, we use dynamic causal modeling (DCM) for fMRI to investigate effective connectivity between dMPFC and TPJ in schizophrenia patients with computerized reference community task, which required participants to understand others' mental states and apply their social knowledge of others' mental states. We also examine the intervention effects on fronto-temporal effective connectivity from Social Cognition and Interaction Training (SCIT) for schizophrenia.We believe this study will lead to a better understanding of neurobiological basis of theory of mind deficits in schizophrenia. Also, this study will provide theorical evidence of clinical implication of SCIT in schizophrenia.
