中文摘要: 已有的医学影像方法在术前诊断和术后评估方面发挥了重要的作用,然而随着医学影像学技术的发展,精准医学的临床需求对疾病的诊疗提出了更高的要求。如何早期发现微小肿瘤病灶,如何术中快速、高特异地切除肿瘤组织成为了临床医学领域亟需解决的挑战性问题。基于解剖结构及功能代谢的影像学方法难以应对新挑战。光学分子影像作为新兴的医学影像方法在这方面显示出巨大的潜力,然而在现有光学分子影像方法的应用中,正常组织对成像造影剂的非特异性摄取影响了成像效果,限制了临床的广泛应用。因此为提高光学分子影像方法检测乳腺肿瘤的特异性,实现肿瘤靶分子的准确定量分析,本课题拟从成像方法入手,结合前期研发的光学分子影像手术导航系统,通过注射乳腺肿瘤目标外的非靶向造影剂,校正正常组织对靶向造影剂的非特异性摄取,实现术中对乳腺癌早期微小肿瘤病灶高特异的成像,辅助外科医生实施精准手术切除,为精准医学的临床需求提供一种有效的解决方法。

英文摘要: Current medical imaging methods have played an important role in the preoperative diagnosis and postoperative evaluation. However, with the development of medical imaging technology, a higher standard for precision medicine is demanded for the treatment of clinical needs. How to detect small tumor lesions in early stage and how to determine the tumor tissue fast and with high specificity during surgery became challenging problems. The current imaging methods based on the anatomy and function faced the difficulty to cope with the new challenges in clinics. Fortunately, molecular imaging as a new medical imaging method shows great potential in this regard. However, the conventional molecular imaging method used tumor specific probes has the limitation of the non-specific uptake of the normal tissue, which limit its widespread application in clinical practice. Therefore, in order to improve the precision of breast tumor-specific imaging and accuracy of quantitative analysis in tumor target molecule, we propose an novel imaging method with high specificity based on our previous surgical navigation system. In order to realize breast cancer-specific imaging, we inject the tumor-specific and non-targeted agents together to achieve tumor correction in early stage breast cancer imaging. Our novel imaging method will assist surgeons precisely detect small breast tumor and resect specifically during operation. Our imaging method will provide an effective solution to the clinical needs in precise medicine.
