中文摘要: 左心室涡流可能是心肌疾病的发生原因,也可能是心肌疾病导致的结果,其发生机理和病理生理意义值得深入研究。本项目拟用MRI技术研究左心室涡流的检测与表达方法。以申请人在黑血对比双反转快速FSE序列(DIR-FSE)发现的左心室内血流高信号代表涡流为原创发现进行探索性研究,以超声向量血流成像(VFM)显示的左心室涡流特征为参照,探讨DIR-FSE序列心室高信号、流速编码相位对比序列(VEC-PC)显示左心室涡流的方法和定性、定量表达方式,结合心脏常规MRI和超声速度向量成像(VVI)检查,选择心肌致密化不全、扩张性心肌病、心肌梗死和高血压病例,分别代表心肌结构异常、左心室几何形态异常、心肌运动异常和左心室前后负荷异常,探讨MRI显示左心室涡流及其病理生理机制与诊断价值。

英文摘要: Left ventricular (LV) votex may result from or result in some kinds of myocardial diseases and further investigation of its mechanism and pathophysiology is worthwhile.In this study, we focus on the detection and expression of the LV vortex using MRI techniques. This idea was originated from the clinical finding of unsupressed blood signal inside the left ventricle on black blood double inversion recovery fast spin echo sequence (DIR-FSE), which showing high signal in subendocardial region and may result from the abnormal trubulent flow secondary to some myocardial diseases. The parameters measured by ultrasound vector flow mapping (VFM) was taken as reference standard for LV vortex. The occurrence and signal pattern of LV high signal on DIR-FSE sequence, the flow pattern on velocity-encoding phase contrast sequence (VEC-PC) are quantitatively and qualitatively correlated with the parameters measered by ultrasound VFM, velocity vector imaging (VVI) and MRI measurements. The patietes of LV noncompaction, idiopathic dilated cardiomyopathy, myocardial infarction and hypertension, as the examples of myocardial structure, morphology of LV, myocrdial function,pre- and post- LV load anaomalies, respectively, will be included for the evaluation of the mechanism and pathophisiology as well as the diagnosis efficiency of the LV vortex detected by the MRI techniques.
