中文摘要: 手术疗效预测是颞叶癫痫外科治疗管理流程中的一个关键环节,也是当前癫痫影像诊断亟需加强的研究目的之一。探索影响手术疗效的病理生理机制,以及发展可靠易行的疗效预测方法具有重要意义。本项目拟提出影响手术疗效的患侧颞叶外广泛脑区参与网络理论,通过静息态功能磁共振成像局部活动及脑连接网络分析方法,对颞叶癫痫手术前后脑功能改变的模式进行纵向观察,寻找不同疗效患者脑功能网络重组中关键性脑区及网络指标。本项目揭示手术干预对颞叶癫痫脑功能网络可塑性机制,开发病灶负载条件脑网络分析新方法,为颞叶癫痫手术疗效预测提供客观的影像学指标,拓宽癫痫手术临床评价体系。

英文摘要: Predicting surgical outcome is key to surgical treatment and management of temporal lobe epilepsy. It is the one of purposes of neuroimaging diagnosis of epilepsy, which urgently needs to be strengthened. It is important clinical relevance of surgical outcome prediction that exploring the pathophysiological mechanism and developing reliable and feasible approaches. We aimed to propose a new conception that epileptic network can spread well beyond the anatomical or even pathological defects in temporal lobe. More important, this epileptic network would affect the surgical treatment. To this end, we used the local brain activity and distant functional connectivity emerging as brain connectome by resting-state functional MRI. By this means, we would observe a longitudinal brain activity before and after epilepsy surgery. In addition, we would detect the 'hub' regions and network attributes to describe the brain network reorganization in patients with different postsurgical outcome. Furthermore, we might uncover how surgical intervention sculptures the brain function connectome plasticity mechanism. In the current project, we propose a new method to investigate the lesion load brain functional network, provide a objective neuroimaging measurements for predicting temporal lobe epilepsy surgery outcome, resulting in a complete epilepsy surgery clinical evaluation system.
