中文摘要: 阿尔茨海默病(AD)是威胁老年健康的脑部重大疾病,遗忘型轻度认知障碍(aMCI)是AD的早期阶段,约有一半以上aMCI进展为AD,AD的早期诊断对治疗干预可能会有重要意义。磁共振结构和功能成像是无创性的检查技术,为检测MCI及AD的脑动态变化提供了重要手段。进展性aMCI及AD患者的脑结构及功能衰退较正常老年呈现为加速、进展性改变。本课题应用磁共振3D T1结构及3D 动脉自旋标记(ASL)脑灌注成像动态跟踪记录aMCI、AD及正常老年组基点、1年、2年后的脑图像,定量跟踪aMCI和AD脑变化的趋势,将脑区按照AAL 脑模板分区为感兴趣区,利用支持向量机的机器学习方法分析提取aMCI及AD脑较正常对照加速的脑结构和功能衰退特征,构建纵向变化模型和生物标志,用于识别aMCI、AD及正常老年,并对进展性aMCI患者进行预测。研究结果会有助于促进AD的早期诊断,有较强的临床实用意义。

英文摘要: Alzheimer's disease (AD) is an aging-related brain disorder that seroiusly threatened elderly people's health. Amnesic mild cognitive impairment (aMCI)belongs to preclinical stage of AD, and about half of aMCI patients will transfer to AD. Early diagnosis of AD may help for ealy intervention. As a non-invasive method, MRI strucrtural and functional imaging provide a multi-modality tool for dynamic recording brain changes. Structural and funtional brain changes in AD and MCI present accelerated aggravation compared those in normal elder adults. In the present study, tructural 3D T1 weihgted image and 3D arterial spin labelled image (3D ASL) were applied for dynamic recording brain changs in aMCI, AD and normal elderly groups at basline, post one and two years. Brain image will be segmented according to AAL template. Then we use machine learning method based support vector machine to identify and extract features of the stucrtural and functional images,and reconstruct longitudinal brain moldel to select effective AD biomarker, and robust AD and MCI classifier will thus be constructed. the biomarkers will also be uesed to identify the progressive MCI individuals and predict for early AD. The presnet research will help for early diagnosis of AD with strong practical clinical significance.
