中文摘要: 海洛因成瘾是一个复吸率极高的慢性脑病,美沙酮维持治疗能有效降低其复吸率。目前美沙酮维持治疗对海洛因成瘾患者静息态脑功能和白质微结构的影响尚不清楚。我们的前期研究表明慢性海洛因滥用能影响海洛因成瘾患者静息态脑功能(文章发表在RADIOLOGY)和白质微结构的完整性,美沙酮维持治疗对其功能的恢复起到部分代偿作用(发表在NEUROIMAGE),然而其确切的神经机制尚不清楚。本项目在此基础上拟采用BOLD-fMRI、DTI和海洛因渴求问卷(HCQ)对比海洛因成瘾患者美沙酮维持治疗前和治疗后6个月静息态脑功能、白质微结构及海洛因渴求程度的变化,探讨美沙酮维持治疗对海洛因成瘾患者脑功能及白质微结构的影响,从而明确美沙酮维持治疗的神经机制。以期为慢性海洛因成瘾患者美沙酮维持治疗的疗效评估、预后判断提供客观影像学依据。该研究是一个涉及社会学、成瘾医学、医学影像学、信息学和心理学等多个学科的多层次交叉研究。

英文摘要: Heroin addiction is chronic relapsing disease of the brain. Methadone maintained treatment (MMT) can prevent from drug craving and reduce the relapse rates effectively. However, the exact neurobiologic mechanisms underlying MMT on the brain of heroin dependent individuals (HDI) are still not fully understood. Our previous work had indicated that chronic heroin abuse can disrupt the resting-state brain function (Article published in RADIOLOGY) and white matter microstructure in HDI, while methadone can ameliorate this effect (Article published in NEUROIMAGE). Based on our preliminary work, the blood oxygen level dependent (BOLD) functional magnetic resonance imaging (fMRI) in resting state, diffusion tensor imaging (DTI) and heroin craving questionnaire (HCQ) are performed to investigate the changes of brain function, white matter microstructure and heroin craving in HDI before and after MMT. The purposes of this proposal are to assess the relationship between these brain functional, structural changes and the dosage of methadone use as well as heroin craving in HDI before and after MMT; to investigate the neural mechanisms of MMT on heroin craving in HDI.
